Se han revisado 48 casos donde se implantó un vástago de anclaje metafiso-diafisario con recubrimiento total de hidroxiapatita en cirugía de revisión aséptica de la cadera. En 9 casos el defecto se clasificó como de tipo 1, en 27 era de tipo 2 (12 de tipo 2-A, 6 de tipo 2-B y 9 de tipo 2-C) y en 12 de tipo 3. El periodo de seguimiento ha oscilado entre 1 y 7 años con una media de 3,9. En 11 casos se digitalizaron las imágenes obtenidas radiográficamente mediante la captura con una cámara digital, procediéndose después a su tratamiento informático y a ecualización por histograma. No se apreciaron hundimientos mayores de 2 mm en ningún paciente ni movilizaciones varovalgo. En 32 casos era apreciable una osteoporosis en la zona del trocánter mayor al año de la intervención que se fue modificando con el paso del tiempo de manera irregular y sin pautas uniformes. Al año se observó en todos los pacientes un adecuado relleno de los espacios vacíos metafisarios y diafisarios, con una excelente integración de los aloinjertos. Mediante la digitalización de imágenes se comprobó la recuperación de la estructura ósea próxima al vástago así como la ausencia de líneas sospechosas de fijación fibrosa. |
Summary: |
We analysed the results of forty eight hydroxyapatite coated femoral component implanted in aseptic revision of the hip. In 9 cases the defect was classified as type 1, in 27 of type 2 (12 of type 2-A, 6 of type 2-B and 9 of type 2-C) and in 12 of type 3. The follow-up was between 1 and 7 years with a mean of 3,9. In 11 cases the x-rays were analysed by image capture with a digital camera, being processed later with a computer and equalization by histogram. No subsidence bigger than 2 mm was observed in any patient, neither displacement varus-valgus. In 32 there was cases an osteoporostic area in the area ogreatter trochanter a year after the intervention; that was irregularity they modified over the time. At the first postoperative year an appropriate filling of the empty metaphyseal and diaphyseal spaces was observed in all the patients, with an excellent integration of the allografts. With digital images, the recovery of the next bony structure as well as the absence of suspicious lines of fibrous fixation was proven. |
