Resumen:
La sinovitis villonodular pigmentada (SNVP) es una enfermedad benigna pero localmente agresiva de proliferación sinovial que puede ocurrir en todas las articulaciones, vainas tendinosas y bursas, con predilección por la rodilla. La etiología de PVNS sigue sin estar clara. La resonancia magnética es el mejor método diagnóstico para establecer la sospecha de SNVP aunque el gold standard para el diagnóstico es la biopsia sinovial. El tratamiento inicial se basa en la sinovectomía, abierta o artroscópica. Tras la cirugía, existe una alta tasa de recurrencia. La sinoviortesis radioisotópica como adyuvante a la sinovectomía quirúrgica parece disminuir la tasa de recidiva local. Se presenta el manejo y la evolución de un caso de SNVP de rodilla mediante sinovectomía artroscópica y radiosinoviortesis adyuvante mediante la administración intraarticular de 5 mCi de Y-90 junto a 0.5 mCi de 99mTc
Summary:
Pigmented villonodular synovitis (SNVP) is a benign but locally aggressive disease of synovial proliferation that can occur in all joints, tendon sheaths and bursae, with a predilection for the knee. The etiology of PVNS remains unclear. Magnetic Resonance Imaging (MRI) is the best diagnostic method to establish suspected SNVP, although synovial biopsy is the gold standard for diagnosis. Initial treatment is based on synovectomy, open or arthroscopic. After surgery, there is a high rate of recurrence.1 Radioisotope synoviotrosis as an adjunct to surgical synovectomy appears to decrease the rate of local recurrence. Management of a case of knee SNVP by arthroscopic synovectomy and adjuvant radiosynoviordesis is presented.
