Resumen:
Introducción: El objetivo era comparar el daño tisular ocasionado por el abordaje anterior y el lateral para una prótesis total de cadera. Métodos: Estudio prospectivo de cohortes comparando pacientes intervenidos de prótesis total de cadera mediante abordaje anterior o lateral. El daño tisular se evaluó mediante niveles séricos de interleucina, factor de necrosis tumoral y reactantes de fase aguda, y resonancia magnética. La función se evaluó con la escala de cadera de Harris. Resultados: Los parámetros de daño tisular fueron significativamente más elevados precozmente con el abordaje lateral, aunque igualándose hacia los 30 días postoperatorios. No había diferencias significativas del resultado funcional a los 3 y 12 meses. Conclusión: Aunque el abordaje lateral presenta una mayor agresión tisular en el postoperatorio precoz, el resultado funcional y la velocidad de recuperación funcional a corto plazo son similares entre ambos abordajes
Summary:
ntroduction: The objective was to compare the tissue damage caused by the anterior and lateral approaches for a total hip replacement. Methods: Prospective cohort study comparing patients undergoing total hip replacement using an anterior or lateral approach. Tissue damage was assessed by serum levels of interleukin, tumor necrosis factor, and acute-phase reactants, and magnetic resonance imaging. Function was assessed with the Harris hip scale. Results: The parameters of tissue damage were significantly higher early with the lateral approach, although equalizing towards 30 postoperative days. There were no significant differences in functional outcome at 3 and 12 months. Conclusion: Although the lateral approach presents greater tissue damage in the early postoperative period, the functional outcome and speed of recovery in the short-term are similar between both approaches.
