Resumen:
La conservación de la línea articular de la rodilla en su posición anatómica es de gran importancia tras una artroplastia primaria. Objetivo. El propósito de esta revisión bibliográfica y de parte de nuestra casuística, es determinar la referencia ósea más apropiada para el cálculo de la línea articular. Material y métodos. Estudio retrospectivo de 30 pacientes con artrosis de rodilla. Radiografías de frente y perfil. La línea articular fue determinada a partir de las siguientes referencias: Epicóndilo medial, y lateral, tubérculo aductor, “lateral flare”, tubérculo tibial anterior y cabeza del peroné. Resultados. La muestra estaba compuesta por 23 mujeres y 7 hombres, con una edad media de 71,3 años (± 6,34). Veintiocho prótesis eran PS y solo 2 CR. La línea articular no se vio modificada en gran medida tras artroplastia primaria. No se encontraron diferencias significativas que pudiesen ser atribuidas a la edad, sexo o modelo de prótesis. El tubérculo aductor y la tuberosidad tibial anterior fueron las referencias más precisas (83% y 70% casos) Observamos diferencias estadísticamente significativas para las medidas absolutas en relación con el sexo, que se corrigieron con la utilización de ratios. Conclusiones. La línea articular se encuentra restituida en la mayor parte de casos tras ATR. La referencia ósea del tubérculo aductor, es la más fácil y reproducible de emplear en la práctica clínica. El empleo de los ratios femoral y tibial permiten corregir variaciones debidas al tamaño femoral y tibial de cada paciente y a los errores de magnificación de la imagen.
Summary:
Joint line preservation is of great significance in knee replacement. Purpose. Bibliographic review and own cases revision to establish proper bone references to assess joint line Material and Methods. Retrospective assessment of 30 patients with knee osteoarthritis. A-P and lateral X-ray were evaluated. Joint line was measured from next references: medial and lateral epicondyle, medial and lateral flare, anterior tibial tubercle and head of the fibula. Outcomes. 23 women and 7 men of 71.3 (± 6.34) of age were included in this revision. Twenty-eight PS prostheses and 2 CR were assessed. No changes were observed in joint line after primary knee replacement. No big differences were related to age, sex or prostheses design. Medial flare and anterior tibial tubercle were the most precise references (83 and 70 % of cases). We found differences with statistically significance for absolute measures related to sex that were matched with the use of ratios. Conclusions. Joint line is re-established in the majority of cases after primary knee replacement. Medial flare is the easiest and reproducible bone reference in clinical practice. Femoral and tibial ratios allow mending possible changes due to sizes of the patient and mistakes related to magnification of the image.
