Resumen:
La disociación espino-pélvica es una lesión infrecuente, aunque probablemente infradiagnosticada. Se define como una fractura del sacro que combina un trazo transversal con trazos sagitales en ambas alas sacras, provocando una separación mecánica entre el raquis y la pelvis. Es fundamental mantener la sospecha ante un traumatismo de alta energía. El tratamiento de elección, salvo excepciones, es quirúrgico. Como complicaciones asociadas, debemos determinar la afectación neurológica prequirúrgica de estos pacientes fundamentalmente, puesto que será el principal determinante del pronóstico a largo plazo. El objetivo de este trabajo es dar a conocer una patología poco habitual, pero con importantes consecuencias a través de un caso clínico.
Summary:
Spinopelvic dissociation is a relatively unknown injury, probably underdiagnosed. The term is applied to those fractures of the sacrum which combines a transverse fracture with sagittal fracture in both sacral wings, resulting in a mechanical separation between the spine and the pelvis. It’s really important to be alert in a high energy multiple trauma. The treatment of choice, except exceptions, is surgery. About associated complications, we must detect preoperative neurological injury of these patients due to it will be the main determinant of the long-term prognosis. The objective of this review it is to introduce this rare pathology with important implications through a clinical case.
