Se estudia el tratamiento de las fracturas inestables del macizo trocantéreo en pacientes ancianos mediante el clavo cóndilo-cefálico de Kuntscher. Han sido tratados y controlados 28 pacientes a los que no se les ha permitido apoyar hasta el final de la consolidación para evitar; la penetración del clavo en cadera o su migración distal. Esta serie es comparada con otra análoga de pacientes que apoyaron poco después de la osteosíntesis. Los resultados revelan la conveniencia de evitar el apoyo para conseguir una consolidación favorable sin complicaciones de la síntesis y los efectos nocivos del apoyo precoz causante de migraciones proximales y distales del clavo en más de la mitad de los casos. |
Summary: |
The treatment of the unstable fractures of the trochanteric region of the femur in elder patients by means of the ondylo-cephalic nail of Kuntscher. Is considered 28 patients have been treated and controlled who have not been allowed to begin weight=bearing till the end of the fracture healing to avoid the penetration of the nail into the joint cavity through the femoral head or its distal migration. This series is compared with another one similar of patients who began weight-bearing soon after the osteosynthesis. The results prove the advantage of avoiding weight=bearing to obtain a correct bone healing without complications of the synthesis and the harmful effects of the precocious weight=bearing, which is the cause for proximal and distal migrations of the nail in more than half of the cases |
