El tratamiento quirúrgico de la artritis reumatoidea (P. R.) en la cadera es la al troplastia de sustitución, pero presenta ciertas particularidades propias de la índole de la enfermedad reumática de la afectación poliarticu1ar en el miembro inferior que hace que plantee problelnas no habituales en la artroplastia de cadera por otras indicaciones (artrosis, necrosis cefálica, etc.). La lesión de la articulación, la osteoporosis, la edad de los enfermos, el grado de invalidez, hace que varíen sus indicaciones. Asimismo la técnica por la osteoporosis ósea y la tendencia a la protusión acetabular plantea problemas para la fijación de los componentes acetabular y femoral. El riesgo de infección, flebotrombosis, osificaciones postraumáticas, es distinto. Los autores analizan según su experiencia estos problemas, así como la técnica seguida en la cirugía de la cadera de la artritis reumatoidea. |
Summary: |
The surgical treatment in the reurnathoid arthritis of the hip is the total hip replacement; however, its problems eiffer from those of the total hip replacement in other diseases (ostenarthritis idiopatic necrosis of the head of the femur, acetabulum fractures). Age and afectation of someother joints of the lower limb, osteoporosis and fhe late risk of protusion of the hip prothesis have their special problems. The authors report their experience on the indicatiüns and technique, operative details as well as the new trends in this surgery. |
