Los autores aportan su experiencia según la casuística de 256 casos de traumatismos cervicales asistidos en los últimos 7 años y medio, desde 1976 a 1983, 60 de los cuales fueron intervenidos (23'4 por 100). La complejidad de estos traumatismos viene determinada por su mecanismo y la frecuencia de las complicaciones neurológicas, medulares y radiculares que plantean una fisiopatología propia y unas posibilidades terapéuticas. Se juzga el concepto de inestabilidad y las indicaciones de estabilización quirúrgica. En la experiencia de los autores, la artrodesis atloaxoidea para las lesiones de este nivel y la artrodesis con la técnica de Rogers para las fracturas-luxaciones inestables de C3 y C7 son las de elección. Para el resto de los traumatismos cervicales inestables se aconseja la vía anterolateral, artrodesis intersomática tipo Robinson o la osteosíntesis con placa doble en T tomillada (tipo Senegas). Se valoran los resultados de los 60 casos intervenidos. |

The authors report their experience on 257 cases os cervical spine dislocation-fractures treated along the last 7 1/2 years (1977-83), 60 of them being operated (23'4%). The mechanism and frequency of the neurological complications (spinal cord and roots) gives way to its own physiopathology and treatment possibilities. The stability or non-stability of this injuries is the main feature in order to perform a surgical fixation. In the author's experience the atloaxoidea arthrodesis is the best method for injuries at this level and the posterior arthrodesis following Roger's technique for unstable dislocations- fractures L3/L7. The anterolateral approach is used for someother unstable fractures of the cervical spíne, using the inter-somathic arthrodesis according Robinson's technique or internal fixation with screwed double T plate (Senegas procedure). . The results obtained on the 60 operated cases are collected. |
